Registration Forms
Each participant in Cornerstone Athletics camps and clinics must complete a Registration Form, Liability Form, and Consent/Decline to Photograph Form located below. Registration will not be complete until all forms and payments are received. A participant’s mailed registration will be considered “late,” with additional late fees applied, if it is postmarked after the published due date for the camp or clinic. We accept checks and money orders made out to Cornerstone Athletics (please be advised that writers of returned checks will be responsible for all bank charges). Registrations for the Winter Skills Clinics must be postmarked by December 24. Registration will close when each session has 30 children enrolled or on December 24, whichever comes first. Any registrations received after sessions close due to full capacity will be placed on a wait list. In the event of a cancellation, persons on the wait list will be contacted on a first-come, first-served basis.
